This Website Privacy Policy explains how CareContinuum handles personal information when you visit this website, request early access, or otherwise contact us through the website.

This policy covers the public CareContinuum website and early-access form. It does not describe the privacy practices of a future CareContinuum product, which will require separate disclosures before launch.

CareContinuum is designed to support caregiver coordination and does not replace healthcare professionals or clinical judgment.

Information we collect

When you request early access, we collect the information you choose to provide through the form:

• Your selected perspective, such as family caregiver or healthcare professional.
• Your selected reason for requesting early access.
• Campaign parameters, landing-page URL, and referring-page URL when available.

The website does not currently provide user accounts, accept payments, or ask for diagnoses, medication details, discharge documents, or other health information.

How we use information

We use early-access information to:

• Record and respond to early-access requests.
• Understand interest in CareContinuum from different caregiver and care-community perspectives.

We do not use information submitted through this website to diagnose, provide treatment recommendations, change medication instructions, or make clinical decisions.

Health and sensitive information

Do not submit medical records, diagnoses, medication information, discharge instructions, Social Security numbers, payment information, or other sensitive information through the public early-access form.

The public website and early-access form are not intended to receive protected health information and are not a substitute for contacting a healthcare professional or care team.

Browser storage and website technology

The website uses browser session storage to remember campaign parameters, the first landing page, and the referring page during your current browser session. This information is attached to an early-access submission if you send the form. Session storage normally clears when the browser tab or session ends.

CareContinuum does not currently place advertising or analytics cookies through this website. The site loads fonts from Google Fonts, which means your browser connects to Google when those font files are requested. Your browser and hosting providers may also process standard technical information, such as an IP address, browser type, requested page, and request time, to deliver and secure the website.

Service providers and disclosures

The early-access form is processed by Formspree. Information submitted through that form is sent to Formspree so it can deliver the submission to CareContinuum. Formspree handles information under its own privacy policy.

We may also use hosting, infrastructure, communications, security, or professional service providers when reasonably necessary to operate the website and respond to requests. These providers may process information only as needed to perform their services.

We may disclose information when required by law, to respond to lawful requests, to protect rights and safety, or as part of a business transaction subject to appropriate safeguards. We do not currently sell personal information collected through this website or use it for cross-context behavioral advertising.

International processing

CareContinuum’s website service providers may process information in countries other than the country where you live. Privacy and data-protection laws may differ between jurisdictions.

Where required, CareContinuum will use appropriate measures for international transfers of personal information.

Your choices and privacy rights

You do not have to submit the early-access form. Depending on where you live, you may have rights to request access to, correction of, or deletion of personal information, or to object to or restrict certain processing.

To make a privacy request, email info@carecontinuum.ca and identify your message as a privacy request. Please do not include health or other sensitive information. We may need enough information to verify the request and will respond as required by applicable law.

You can prevent the site from retaining campaign attribution during a session by blocking browser storage or clearing site data in your browser settings. Some attribution fields may then be unavailable.

Retention and security

We retain early-access submissions only for as long as reasonably necessary to manage the waitlist, communicate about CareContinuum, satisfy legal obligations, resolve disputes, and protect the website and our rights.

We use reasonable administrative and technical measures appropriate to the information handled by this website. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

Children

The website and early-access form are not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this website. If you believe a child has submitted information, contact us so we can review and, where appropriate, delete it.

Changes and questions

We may update this Website Privacy Policy as CareContinuum and its website develop. We will post the updated policy here and revise the effective date when changes are made.
