1. Agreement to Terms

These Terms govern your access to and use of the public CareContinuum website and its early-access form. By using the website, you agree to these Terms and our Privacy Policy. If you do not agree, do not use the website.

These Terms do not govern a future CareContinuum product, caregiver application, or clinical service. Separate terms will be provided before any such service becomes available.

2. Website Content and Intellectual Property

The website and its text, graphics, logos, interface examples, illustrations, and other content are owned by CareContinuum or used with permission and are protected by applicable intellectual-property laws.

You may view and use the website for personal, informational, and non-commercial purposes. You may not reproduce, distribute, sell, modify, or exploit substantial portions of the website without permission, except where applicable law permits.

3. Eligibility and User Representations

You represent that you can legally agree to these Terms and that information you submit is accurate, belongs to you, and does not violate another person’s rights. The website is not directed to children under 13.

4. Early Access and No Fees

Submitting the early-access form expresses interest only. It does not create an account, guarantee access, establish a customer relationship, reserve a launch date, or obligate CareContinuum to provide a product or service.

The current website does not charge fees, accept payment information, offer subscriptions, or sell access. Any future paid offering will have separate pricing and terms presented before purchase.

5. Stopping Use

You may stop using the website at any time. Because the current website does not provide user accounts or subscriptions, there is no website account to cancel. Privacy requests concerning an early-access submission are described in the Privacy Policy.

6. Prohibited Activities

You may not misuse the website or attempt to interfere with its operation. Prohibited conduct includes:

  • Violating applicable law or another person’s rights.
  • Submitting fraudulent, deceptive, threatening, unlawful, or harmful material.
  • Attempting unauthorized access, security testing, disruption, or circumvention.
  • Introducing malicious code, automated spam, or unreasonable traffic.
  • Scraping or copying substantial website content except as permitted by law.
  • Misrepresenting an affiliation with CareContinuum or another person.

7. Information You Submit

The early-access form accepts basic contact and interest information. Do not submit medical records, diagnoses, medication information, discharge instructions, government identifiers, payment details, or other sensitive information.

You remain responsible for information you submit and must have the right to provide it. Our handling of personal information is described in the Privacy Policy.

8. Feedback

If you voluntarily provide product ideas or feedback, you permit CareContinuum to use that feedback without restriction or compensation to evaluate and improve its work. This does not transfer ownership of your personal information or confidential materials.

9. Website Management

We may update, suspend, restrict, or discontinue any part of the website; correct errors; remove content; or block access when reasonably necessary for maintenance, security, legal compliance, or protection of the website and its users.

10. Term and Termination

These Terms apply while you use the website. We may restrict or terminate access if we reasonably believe you have violated these Terms or created a security, legal, or operational risk. Provisions that by their nature should survive will remain effective.

11. Applicable Law

These Terms are governed by applicable law. CareContinuum has not designated a specific legal entity, governing jurisdiction, or exclusive venue in these website Terms. Those details must be added before commercial launch where legally required.

12. Disputes

Before beginning formal proceedings, you and CareContinuum should attempt in good faith to resolve concerns through the contact method below. Nothing in these Terms requires arbitration, waives class-action rights, or selects an exclusive court or venue.

13. Healthcare and Safety Disclaimer

The public website provides general information about CareContinuum’s intended caregiver-coordination purpose. It does not provide medical advice, diagnosis, symptom interpretation, clinical scoring, medication recommendations, dosing guidance, treatment, or emergency services.

Website examples are illustrative and are not discharge instructions for any person. Care guidance and clinical judgment remain with qualified healthcare professionals. Do not use the website as a substitute for contacting the appropriate healthcare professional or care team.

14. Disclaimer of Warranties

To the extent permitted by law, the website and its content are provided “as is” and “as available.” We do not warrant that the website will always be uninterrupted, error-free, secure, complete, or current. Nothing on this website is a promise that a displayed or described feature is currently available.

15. Limitation of Liability

To the extent permitted by applicable law, CareContinuum will not be liable for indirect, incidental, special, consequential, or punitive damages arising from use of or inability to use this public website. Nothing in these Terms excludes liability that cannot legally be excluded or limited.

16. Privacy and Data

Our Privacy Policy explains the website information we collect, browser session storage, Formspree processing, retention, security, and privacy choices. The current website does not provide a patient portal, electronic medical record, or clinical record system.

17. Electronic Communications

By submitting an early-access request, you agree that CareContinuum may contact you electronically about that request and related product research or availability. You may ask us to stop non-essential communications. Standard internet communications are not guaranteed to be secure.

18. Third-Party Services and Links

The website uses third-party services, including Formspree for form delivery and Google Fonts for typography, and may link to external websites. Third-party services are governed by their own terms and privacy practices. CareContinuum does not control and is not responsible for independent third-party content or practices.

19. Miscellaneous

These Terms and the Privacy Policy are the entire agreement concerning use of the current public website. If a provision is unenforceable, it will be limited to the minimum extent necessary and the remaining provisions will continue. A failure to enforce a provision is not a waiver.

We may update these Terms as the website develops. Updated Terms will be posted here with a revised effective date. Materially different product functionality may require separate terms rather than an amendment to these website Terms.
